Dr. Packard has retired, and Pediatric Psychology of Dayton has closed.

Records may be obtained by contacting Dr. Packard at packard@thomaspackardphd.com. For former patients who are still under the age of 18, requests must be made by a parent or guardian. Patients who are now 18 years old or older must request records themselves.

Ohio law requires that records for minor patients be maintained until the patient is 2 years past the age of majority, or until 7 years after the last contact between the psychologist and the patient. If you are over the age of 20, and it has been more than 7 years since our last contact, all records have been destroyed in accordance with the law.
